Two matching white labels, each headed by the America West Airlines logo, leave ruled lines for a traveler to fill in name, address, city, state, zip and phone before sticking them to a bag. America West was founded in February 1981 and began flying on August 1, 1983, with three leased Boeing 737s from its hub at Phoenix Sky Harbor, later adding Las Vegas. The airline merged with US Airways, announced in 2005, and the two operating certificates were combined in September 2007. Humble pieces like these rarely survive unused.
